Doesn't a real OCD case have only a few of Monk's traits?
Monk seems to have almost every textbook OCD problem. But don't most
people only have 2 or 3 of those?

Some OCD people have every symptom in the books, sometimes all at once, sometimes
in a somewhat mysterious series of rotations. (Germophobe and counter today through Tuesday,
checker after that for a few days, with overlapping days of handwashing, etc.)

They should invent an OCD remote control for loved ones. Leaving on
vacation tomorrow? Switch it to the Checker channel. The house a little
untidy? Click on compulsive cleaner...

All I know is, my wife (who has exceptionally mild symptoms in comparison to
Monk) loves the show largely because she agrees with everything he finds
gross . . . .

Reality is, most have a few, few have only one "major" area, almost none
have as many as he does.
However, he's ALOT more realistic than the char. in As Good As It Gets.
It was a good flick, but his OCD was HORRIBLY unrealistically portrayed.

In fact, I've NEVER heard of it, and I've read books on the subject and
encountered probably more people with it than anybody else in this group,
unless there's a dr. who specializes in it. BTW, just A dr. doesn't
count. Most are shockingly uninformed about it.

People can have a "paradigm shift" (IIRC that's what it's called) where
their symptoms shift, but it's usually only slightly (from an outsider's
point of view). Like lets say the person's afraid of germs for several
years, then it gradually shifts into asbestos. For a while he will have
both, and then it can go all the way into the new one.
In short shifting is the exception, not the norm.
And, as in the above example it's not that great of a switch.
You would still call it "contamination" type OCD in both cases.

It's rare, but does happen.
